The NetGear B90-755D44-15 is one model of DSL modem used by Frontier Communications for business internet service.
Finding official instructions for this modem online seems to be difficult; I have not been able to find any.
Notes
The modem we received was configured to be accessible at http://192.168.254.254 (user: admin, password: admin). It was also loaded with Frontier-branded software (unlike the screenshots in this example, which are branded with AT&T artwork).
