Overview

Coordinated Health Services, Inc. (CHSI) is a provider of health services in North Carolina. They have offices in Clinton, Durham, Elizabethtown, Henderson, Jacksonville, Kinston, Laurinburg, Lumberton, Morrisville ("corporate office"), Raleigh, and Smithfield.

Available services are, according to their web site:

  • Diagnostic Assessment
  • Community Support (MH/MI)
  • Targeted Case Management (DD)
  • Periodic / Individual Supports (MH/DD)
  • Outpatient Therapy

The Durham Center uses CHSI, among others, for case management services.
