In North Carolina, information about filing a Healthcare Power of Attorney (HCPoA) is found in the "ADVANCE HEALTH CARE DIRECTIVE REGISTRY" section of the Secretary of State's web site.

Process

The following should not be construed as legal advice.

It would appear that the process of creating a HCPoA involves the following steps:

  • Print out copies of the HPoA form (currently last revised in 2007)
    • Be sure to include one for yourself, one for each agent you designate, and one for the registry
  • Fill them out.
  • Get each one notarized.
  • Optional but probably a good idea:
    • Print out a copy of the Registration Form
    • Fill it out and mail it in with one copy of the HPoA and $10
